Submitting this form does not automatically confirm warranty coverage. Black River may request additional information, photographs, video, inspection, or return of an affected component before making a coverage decision.

Registration must have been completed within 30 days of delivery and before the reported issue was discovered.

Before You Begin

Please have the following ready:

• MATRIX® serial number
• Photo of the serial number plaque
• Original proof of purchase
• Original vehicle information
• Photos of the complete topper
• Close-up photos of the affected area
• Video if the issue involves opening, closing, Power Lift operation, movement, noise, or leaking

Do not discard, modify, or permanently repair the affected product or component until Black River has completed its review or provided written authorization.
